Pathway of Change

Scw’exmx Child and Family Service Society’s approach to family concerns or requests encompasses many teams working together.

Scw’exmx Child and Family Services Society has designed and implemented its programs with deep roots in culture and traditions, meant to nurture and develop families and family development. In a reflection of these values, the Community Navigator is now the first point of contact when someone makes an inquiry about a child’s wellness, requests support, or has questions about the programs and services. This position was created to ensure each request is being reviewed from a preventative lens, with elder guidance and community collaboration.

When a call comes in regarding a need for support or a child wellness concern, the Community Navigator will do the background fact finding to ensure they have a clear understanding of the family’s needs. These steps ensure the family receives prevention supports as they work toward the goal of keeping families together, focusing on strengths and continuing to work towards post-traumatic growth.

Once that fact finding has commenced, the Community Navigator’s consultation process begins. The Community Navigator will now receive direction not only from a supervisor, but also an Advisory Committee composed of the Elders Team, SCFSS Team Leaders, and Designated Band Representatives.  This Advisory Community can reflect on the strengths of the family and be creative in their approach to supports.

After this consultation, the next steps are for the Community Navigator to complete any referrals to SCFSS prevention and wrap around supports, community prevention, and community partners.  The Community Navigator will also follow up on those referrals to ensure they are not only available, but also a good fit for the family.
